Spastic occlusion of coronary artery during cryoballoon pulmonary vein isolation

چکیده
Introduction Recently, pulmonary vein isolation (PVI) with cryoballoon (CB) has been widely accepted as treatment for atrial fibrillation (AF). Especially, second-generation CB (Arctic Front Advance, Medtronic, Inc, Minneapolis, MN) has a high cooling effect, which can achieve durable lesions of pulmonary vein (PV) antrum and better clinical outcomes. On the other hand, several complications, such as esophageal erosions and phrenic nerve injuries, have been reported with AF ablation using secondgeneration CB. Lehrmann and colleagues have reported a case of near-fatal coronary artery spasm during PVI using second-generation CB. In this case, the mechanism of coronary artery spasm was speculated as cryoenergyinduced blood cooling, which was attributable to the close proximity between the CB and coronary artery spasm site. We experienced a case in which coronary artery spastic occlusion occurred in the distal right coronary artery during CB application at the left common PV, far from the coronary spasm site.

Direct pressure monitoring accurately predicts pulmonary vein occlusion during cryoballoon ablation.
Cryoballoon ablation (CBA) is an established therapy for atrial fibrillation (AF). Pulmonary vein (PV) occlusion is essential for achieving antral contact and PV isolation and is typically assessed by contrast injection. We present a novel method of direct pressure monitoring for assessment of PV occlusion. Transcatheter pressure is monitored during balloon advancement to the PV antrum. Pressur...
متن کاملAcute Right Coronary artery Occlusion During Cooled-Tip Radiofrequency Ablation of the Cavotricuspid Isthmus
Cooled-tip radiofrequencycatheter ablation of the cavotricuspid isthmus, performed after pulmonary vein isolation, resulted in acute occlusion of the posterolateral branch of the right coronary artery in a 49-year-old male with previously known right coronary artery disease. The occlusion was successfully stented. It is conceivable that previously diseased coronary arteries are more prone to be...
متن کاملSuccessful use of bivalirudin in place of heparin infusion for pulmonary vein isolation using a cryoballoon catheter in a patient with heparin allergy
Current guidelines recommend using bivalirudin as an alternative to unfractionated heparin in patients with heparin-induced thrombocytopenia undergoing percutaneous coronary intervention (PCI). Pulmonary vein isolation (PVI) is a well-established treatment strategy for patients with drug-refractory atrial fibrillation (AF) and requires systemic anticoagulation to avoid thromboembolic events, ge...
متن کاملNear-fatal coronary artery spasm during cryoballoon pulmonary vein isolation: an unreported complication.
A 50-year-old nonsmoker with paroxysmal atrial fibrillation was referred to our hospital for pulmonary vein isolation (PVI). He had a known 1-vessel coronary artery disease. Because of stable angina, a drug-eluting stent had been implanted into the midpor-tion of the left anterior descending artery in 2010. Otherwise he was healthy. His medical treatment included warfarin, sotalol, an angiotens...
متن کاملFailure to Reach the Optimal Temperature During Cryoablation Due to Refrigerant Cylinder Problem
INTRODUCTION Pulmonary vein (PV) isolation is considered as a key to atrial fibrillation (AF) treatment. Cryoballoon ablation is an effective therapy for PV isolation for AF with minimal side effects and was approved by the US Food and Drug Administration (FDA) several years ago. Successful isolation of PVs during cryoablation depends on the balloon temperature and helps in early identification...
